Prescription Request

Fill out the form below, phone the surgery or print out a Repeat Prescription Request Form.

HOW IT WORKS

  • Allow two full working days (48 hours) for repeat prescription requests to be processed and remember to take weekends and bank holidays into account. This will ensure your prescription will be ready to collect directly from your nominated Pharmacy.
  • Prescriptions are emailed digitally to your nominated pharmacy so there is no need to call to the surgery to collect prescriptions.
  • Write down clearly what medications you require. Copy names from your medicine boxes or previous prescriptions, if in doubt bring your medication packs to reception and our team will be happy to help you with your request.
  • Repeat Prescriptions cost €20 for Non-Medical Card Holders. All payments are securely processed by Stripe.
  • All patient data is securely processed - read our privacy policy.

MEDICATION REVIEWS

  • ‍‍Patients on repeat medication will be asked to see a doctor or practice nurse to review the prescription at regular intervals.
  • Please ensure that you book an appropriate appointment to avoid unnecessary delays regarding further prescriptions.

Online Prescription

Once submitted, our reception team will be in touch with you to confirm your prescription details and payment.

Non-GMS card holders will incur a fee for prescriptions.
Prescriptions may not be issued without a medication review.
Online payments are not enabled for repeat prescriptions.
After submitting the form, you will be redirected to Stripe to make payment.
